This position is Monday through Friday. No weekends or major holidays. Provide Supervision to the Direct Service workers, maintain caseload up to a maximum of 30 individuals. Coordinate and complete annual assessment. Assist in the development of planned program activities. Develop and review Individual Support Plans and SEEN Plans. Oversee the daily operation of the program. Coordinate training and development of staff in order to ensure the needs of the individuals served are met. Prepare written reports an maintain records in conjunction with DPW, Chapter 2380 Regulations, Area Agency on Aging regulations, and Community Participation Supports. Participate in interdisciplinary meetings. Must be proficient in Microsoft office- word, outlook and excel.
A Master's degree *and *1 year of work or volunteer experience working directly with persons with disabilities or a Bachelor's degree and 2 years of work experience working directly with persons with disabilities or an Associate's degree or 60 credit hours and 4 years of work or volunteer/life experience working directly with persons with disabilities.
