We are seeking a Program Coordinator to assist in developing and implementing a care coordination model for children with obesity at Boston Children’s Hospital. This model aims to integrate services between providers at Boston Children’s Hospital and community partners to ensure equitable access to and appropriate utilization of services.

The Program Coordinator II will be responsible for:
Assisting in creation of workflows
Cultivating relationships with internal and external stakeholders
Processing referrals from partnering physicians
Assessing patient/family needs to optimize treatment of obesity
Connecting patient/family to nutrition counselors, fitness activities or programming, food assistance programs, and behavioral health specialists
Sharing updates with care team members
Additional responsibilities as needed to improve care coordination within partner clinics
Other project coordination and administrative duties as assigned
To qualify, you must have:
Associates Degree required, Bachelor’s degree preferred
2 years of related experience
Flexibility in work schedule to accommodate patient and family schedules; may include occasional evening or Saturday morning hours
Ability to communicate effectively and with empathy, both orally and in writing
Ability to work independently while following established protocols
Excellent attention to detail
Proficiency in Spanish required
This position is full time (40 hours/week). This position is hybrid (both in-person at the hospital’s Longwood campus and remote.)
