Obtains history andsubjective components of the evaluation Tests and measures patient\'sstrength, balance, range of motion, motor development, sensoryperception, functional capacity, and respiratory and circulatoryefficiency, and records findings to develop or revise treatmentprograms. Observes posture, position, and palpates anatomical structuresPlans and prepares written treatment plan based on evaluation of patientdata Administers manual exercises to improve and maintain functionInstructs, motivates, and assists patient to perform various physicalactivities and in use of assistant and supportive devices such ascrutches, canes, and prostheses Administers treatments involvingapplication of physical agents Evaluates effects of treatment at variousstages and adjusts treatments to achieve maximum benefit Administersmassage, soft tissue mobilization, joint mobilization Administerstraction to relieve pain Records treatment, response, and progress inpatient\'s chart or enters information into computer Instructs patientand family in treatment procedures to be continued at home Confers withPhysician and other practitioners to obtain additional patientinformation, suggest revisions in treatment program, and integratephysical therapy treatment with other aspects of patient\'s health careAssist with the supervisory visits of HHA if indicated Participates inthe PI and CQI process through committees as needed Assist in theorientation and training of new staff Education and Experience Graduateof an accredited Bachelor, Master or Doctorate program for PhysicalTherapy Home health experience is preferred Certification/LicensureLicensed as a Physical Therapist in state of practice Approved CPRcertification required within 90 days Maintain a valid driver\'s licenseand reliable transportation with proof of vehicle insurance whenrequested.
