JOB REQUIREMENTS: As a Physical Therapist (PT) at Westfields Hospital &Clinic, you will perform comprehensive assessment of therapy patient,develop a plan of care, perform treatment as indicated, assesspatient\'s response to treatment, and assist with discharge planning.Duties include: Utilize exercises, manual therapy, modalities, andeducation to improve the patient\'s overall mobility, pain, andfunctional ability. Provide education related to the treatment program,disease process, and prevention of further injuries to the patient,family/caregiver, and other members of the healthcare team. Evaluateinpatient and outpatient physical therapy patients. Establish a plan ofcare and implement treatment, including exercise, manual therapy,modalities, and education. Reassess response to treatment, altering planof care as necessary, to optimize a patient\'s mobility, pain, andfunctional abilities. Assist in discharge planning process. Document onelectronic health system in a timely manner. Communicate with referringpractitioner, nursing unit, and other departments when appropriate.Complete continuing education requirements, and provides staffin-service of material learned.
