Successfully engage 85% of children, youth, and families in Permanency Services.Complete 85% or greater of all SWAN units of service and ensure all units are completed in a timely manner based upon SWAN benchmarks, which are updated weekly in the SWAN portal.Complete units of service according to Benchmark requirements including all outreach for records, interviews, and questionnaires. This includes timely and multiple follow ups on requests.```{=html}

Minimum Qualifications for the *Permanency Services Specialist:*
An individual 21 years of age or older, with the ability to identify with and adapt to the goalsand philosophies of the Permanency (SWAN) and independent living program.They will possess a minimum of a BS/BA in social work or similar degree in human services aswell as one year of related experience in the field.Must have and maintain a valid driver's license including vehicle insurance and inspection onany vehicle used for daily work and transportation of clients.Must receive a passing score on annual motor vehicle clearance to be eligible to operate agencyvehicles.Never have been convicted of any prohibited criminal or child abuse offense.
