Roseburg is located on Interstate 5 in Southern Oregon's scenic Douglas County. The community is nestled in a lush forested region known as the 100 Valleys of the Umpqua. In many places you’ll find it hard to maintain a healthy work-life/personal-life balance because the pace of one is indistinguishable from the other. Not here. Life never stops here but it slows to an easy welcome pace. A traffic jam means you missed the yellow and have to wait for the next green. A terrible parking space is the one across from the restaurant instead of in front of it. Job Summary:

Under the direction of the assigned Therapy Supervisor, the Occupational Therapist provides hands-on direct patient care including evaluation, treatment, education, discharge planning and documentation per department standards as a member of the health care treatment team. This would include coordinating and modifying the patient’s care, treatment, goals and discharge planning recommendations to achieve a maximal level of patient functional independence.

Essential Functions:

Incumbent may have access to restricted or confidential patient information and must comply with the terms of Mercy Medical Center Security Policies as it applies to their job role.

Collaborates with immediate supervisor to establish, define and refine department goals and objectives consistent with facility’s Mission and Values, and demonstrates satisfactory progress towards these goals and objectives.

Promotes positive customer relations with internal and external customers.

Demonstrates respect for and sensitivity to cultural/social differences in interactions with others.

Completes assigned tasks in a timely manner.

Demonstrates initiative and problem-solving skills, using sound judgment.

Assumes additional responsibilities as needed, with little or no direction, and shows initiative in assisting others within the department.

Interviews patient, family, caregivers, significant others to obtain information about prior level of function, home environment, family relationships, health and psychosocial history.

Performs client tests, measurements and evaluations as indicated for the patient’s condition and as per any established best practice guidelines and/or department care standards.

Establishes measureable/functional treatment goals that are realistic and consistent with patient/family/caregiver needs.

Plans and prepares written treatment programs based on evaluation, age-related and diagnosis-related policies. Administers treatments as prescribed in the treatment plan. Evaluates effectiveness of treatment and adjusts the treatment to achieve maximum benefits.

Monitors all patients under his/her care: supervises Certified Occupational Therapist Assistants and/or Technicians and assures appropriate treatment and documentation; submits a modified plan of care to the physician for altered plans of care; reports any adverse reactions to treatment or changes in condition to the physician as indicated.

Institutes or assists with emergency measures for sudden adverse developments such as cardiac arrest or medical condition requiring activation of a code, rapid response team or activation of 911.

Implements and assesses appropriate patient and caregiver education per Plan of Care and phases of progress.

Establishes and maintains collaborative communications with physicians, other healthcare professionals and facility departments. This would include team conferences, ICU rounds, other patient care rounds or assigned meetings.

Consistently meets department productivity standards (70%).

Completes required reports, notes and other client-oriented documentation in a timely manner according to policy and department standards, including G-code functional reporting and associated modifiers.

Complies with changing regulations related to insurance, The Joint Commission, state/federal and other requirements as directed.

Documents medical necessity and skilled need for continued treatments in evaluations/re-evaluations and progress notes.

Meets facility standard for Focus on Therapeutic Outcomes (FOTO) status and discharge surveys (minimum of 10 completed per quarter). Uses FOTO recommendations appropriately or documents reasons for exceptions (i.e. progress to date and expectation of significant progress if patient extended beyond FOTO recommendations). (outpatient)

Enters charges accurately into Meditech or on appropriate charge slip.

Schedules patients in an efficient and appropriate manner, with assistance of support personnel

Qualifications

Required Education and Experience:

Must have graduated from an accredited programRequired Licensure and Certifications:

Current Oregon State licensed Occupational Therapist

Currently on the national registration as an Occupational Therapist

Current AHA BLS certification within thirty (30) days of hire

Required Minimum Knowledge, Skills and Abilities :

Skills to work independently essential.

Ability to speak, read and write English effectively is essential.

Ability to apply technical/professional knowledge as appropriate.

High degree of accuracy is required.

Ability to utilize computer to access e-mail, education and electronic medical record/order entry.

Strong interpersonal and communication skills, both written and verbal.

Preferred Qualifications:

Clinical experience dealing with a broad base rehabilitation facility

Pediatric experience

Pay Range

$36.87 - $53.46 /hour
