Role and Responsibilities : A Certified Home Health Aide p rovides personal care and supportive services to individuals in the community , in the privacy of their home.

　　Essential Functions

　　Provides health - related services as outlined in the plan of care , authorized by a physician , and established by a registered nurse or licensed therapist , under the direction of the Aide Matrix established by the NYS Department of Health.

　　Provides assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.

　　Immediately report changes in client’s condition to supervising professional.

　　Ensure the safety of the client , clie nt’s family and/or the employee.

　　Maintain HIP A A.

　　Complete and submit necessary patient documentation including charting tasks and observations on android device.

　　Attends ongoing in-service programs in a timely manner , to maintain certification.

　　All other duties as assigned.

　　Education Requirements

　　High School diploma or GED preferred .

　　Completion of an approved program of Home Health Aide certification as designated by NYS Department of Health.

　　Qualifications and Requirements

　　Valid Driver's License and reliable transportation.

　　Must have proof of completion (Original HHA or PCA Certificate) of an NYS Dept of Health accredited Training program .

　　Must have worked in a certified HHA capacity within the last 24 months (HCR will verify via the NYS DOH's Home Health Aide Registry) .

　　Work Environment

　　The HHA is primarily in a non-office setting and may be exposed to outdoor conditions.

　　The working conditions are classified as medium work:

　　Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.

　　Physical Requirements
