As a key member of the palliative care team, the Advanced Practitioner (APP) will manage a caseload of heart failure patients across inpatient and outpatient settings. The APP’s impactful contributions include developing and monitoring personalized palliative care treatment plans and collaborating with the cardiology and heart failure teams. In consultation with the physician(s) and care team members, Physician Assistants/Nurse Practitioners develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists. Physician Assistants/Nurse Practitioners provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician. Requirements: * Licensure: Unrestricted State of Connecticut Medical License (or eligible) * Training: Physician Assistant or Nurse Practitioner * Experience: New graduates welcome to apply. Experience in cardiology highly preferred. Preferred Experience:1 years in a cardiology/heart failure setting. Palliative care experience a plus.
