Sovah Health is looking for the ideal candidate who will provide primary or acute healthcare services for certain acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice.

• Obtains patient history and performs assessment via observation, interview and examination.• Documents patient care performed, assessments made, and orders given.• Orders, performs and interprets diagnostic studies.• Performs preventative health assessments, screening, immunizations and care.• Provides direct treatment and management of health conditions, including use of prescriptive authority and referral to other healthcare providers or community resources.• Provides counseling and education of patients and their families/caregivers concerning preventative health and wellness, treatment options, and community resources.• Provides care in the context of promoting age- and culture-appropriate informed choices, shared decision making, and the right to self-determination.

Minimum QualificationsMinimum EducationGraduate of a Nurse Practitioner Program required or Master's in Nursing.Required SkillsRequires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.Certifications:American Heart Association Basic Life Support (BLS) requiredLicenses:Currently licensed as a Registered Nurse in the state of Virginia and licensed by the Virginia Department of Health Professions as a Nurse Practitioner requiredCurrent national certification by the American Academy of Nurse Practitioners, the American Nurses’ Credentialing Center or other credentialing center recognized by the board (See Regulations governing the Licensure of Nurse Practitioners published by the Virginia Board of Nursing/Virginia Board of Medicine) requiredCurrent Authorization to Prescribe by the State of Virginia requiredCurrent Controlled Substance Registration required
