Job Summary
Under general supervision, performs telephone triage under conditions thatmay be demanding, stressful and repetitious. Functions independently tocollect data and make assessments, develop a working diagnosis, determineinterventions and disposition per guidelines. Guides evaluation includinginstructing patient/caller how to evaluate normal/abnormal symptoms,effectiveness of treatment and when to call back. Documentssymptoms/complaints, nursing assessment, advice and patient/callerresponse. Follows policies, procedures, and protocols to ensureconsistency and departmental effectiveness as well as improve health careoutcomes of patients/callers and their access to appropriate health care.
Essential Functions:
Collects patient health data in order to identify relevant and uniquepatient/caller needs. Performs initial assessment for neonatal,pediatric, adolescent, adult, and/or geriatric patients. Evaluatescare advice for appropriateness for age of patient/caller. Differentiatesnormal from abnormal findings, including potential problems. Identifiespatient needs and basic understanding of assessment findings as they relate tothe practice of telephone triage. Acts appropriately in critical situations.Identifies unique learning needs of patient/caller and participates in theireducation through care advice and repeat by patient/caller of understanding.Assesses for signs of abuse and makes appropriate referrals. (35%)Reviews medications/drug dosages and allergies with health history andensures immunizations are up to date as appropriate. Performs as team leaderas assigned to monitor the safety of the call queue and acts as liaison forstaffing or interdisciplinary issues. Acts as resource for other staff.Provides feedback to supervisors as appropriate. Analyzes assessment data todetermine most appropriate guideline selection for patient/caller.Documentation is Clear and concise and reflects Nursing assessment and careadvice given. (35%)Reflects any deviations and reason. Performs QI/documentation/audioaudits. Advises appropriate disposition for guideline selection forpatient/caller. Demonstrates the ability to adjust (override) thedisposition according to the unique needs/situation of the patient/callerand using appropriate Nursing Judgment. Integrates quality improvementactivities into practice. Assists in identifying ways to promote quality careand in collecting data needed to promote process and operational improvements.Integrates QA audit findings into practice. Associate's Degree RequiredWork Experience:
1-3 years Two years of recent nursing experience Required andLicenses:
BLS-Basic Life Support (BLS/CPR) - AHA - American Heart AssociationRequired andRN-Registered Nurse - DORA - Department of Regulatory Agencies RequiredKnowledge, Skills and Abilities:
Provides age appropriate triage and treatment dispositions to all callers fromnewborn to Adult, including OB and Worker's Compensation Injury Reportingrequired.Utilizes computer software and associated programs to triage and send reportsto appropriate agencies or clinics required.Uses nursing judgment as an adjunct to the computer Software Program to reachappropriate dispositions and care advice required.Ability to respond appropriately to emergency situations required.Ability to apply nursing principles, practices and techniques required.Ability to exercise initiative and judgment in se ecting proper treatmentrequired.Ability to use computers and computer systems required.Bilingual skills preferred.Computer skills required.Computer skills preferred.
