JOB REQUIREMENTS: Duties Applies comprehensive knowledge of medicalterminology, anatomy & physiology, disease processes, treatmentmodalities, diagnostic tests, medications, procedures as well as theprinciples and practices of health services and the organizationalstructure to ensure proper code selection. Selects and assigns codesfrom the current version of several coding systems to include currentversions of the International Classification of Diseases (ICD), CurrentProcedural Terminology (CPT), and/or Healthcare Common Procedure CodingSystem (HCPCS). Adheres to accepted coding practices, guidelines andconventions when choosing the most appropriate diagnosis, operation,procedure, ancillary, or Evaluation and Management code to ensureethical, accurate, and complete coding. Also applies codes based onguidelines specific to certain diagnoses, procedures, and other criteria(in inpatient and outpatient settings) used to classify patients underthe Veterans Equitable Resource Allocation (VERA) program thatcategorizes all VA patients into specific classes representing theirclinical conditions and resource needs. Monitors ever-changingregulatory and policy requirements affecting coded information for thefull spectrum of services provided by the VAMC. Timely compliance withcoding changes is crucial to the accuracy f the facility database aswell as all cost recovery programs. Assists facility staff withdocumentation requirements to completely and accurately reflect thepatient care provided; provides technical support in the areas ofregulations and policy, coding requirements, resident supervision,reimbursement, workload, accepted nomenclature, and proper sequencing.Ensures provider documentation is complete and supports the diagnosesand procedures coded. Directly consults with the professional staff forclarification of the confliction or ambiguous clinical data. Reportsincorrect documentation or codes in the electronic patient healthrecord. Expertly searches the patient health record to finddocumentation justifying code assignment based on an expanded knowledgeof the organization and structure of the patient record. Uses a varietyof window based applications in day to day activities and duties, suchas Outlook, Excel, Word, and Access; competent in use of the healthrecord applications (Vista and CPRS) as well as the encoder productsuite.
