DESCRIPTION OF WORKAs a Medical Records Assistant at Norristown State Hospital, you code physician diagnosis and procedures by utilizing the International Classification of Diseases system. Work is performed under the direction of the Medical Records Supervisor.
Duties include completing psychiatric and physical diagnostic recording forms, coding diagnoses on all new admissions and annual reviews, and forwarding to the Program Evaluation Department. You also code diagnoses on physical examinations and composite assessments. In addition, you review all admission records within 7 to 10 calendar days following the initial treatment plan. This review is to verify completeness, timeliness, and accuracy of treatment plan entries and multidisciplinary admission assessments. Deficiencies are recorded in a data collection document and copies are then distributed in accordance with departmental procedures.

One year of clerical experience in a medical records department of a hospital, or ambulatory acute medical care setting or a licensed nursing facility, which has included six months of experience under the general direction or oversight by a registered Medical Records Technician or Administrator; and graduation from high school; orGraduation from a medical secretarial or medical records program; orAny equivalent combination of experience and training.Other Requirements:
