Check us out by clicking onThe Care Team Specialist (CTS) participates as a member of a care teamandsupports high quality patient care including engaging patients inmaintaining their physical and mental well-being.The Care TeamSpecialist performs technical, clinical, and administrative tasks whichcontribute to the smooth workflow and quality patient care experience inthe clinic. Responsibilities include, but are not limited to pre-visitpreparation, monitoring patient flow, rooming patients, obtainingpatient histories and vital signs, managing certain test resultsfollowing protocols, supporting Epic in-basket workflow, returning phonecalls, and assisting with medical examinations and procedures. The CareTeam Specialist assists and collaborates with the Registered Nurses andMedical Providers in the management and coordination of patient care.The CTS staff will participate in the review of team/provider in-basketsand My Chart messages for the assigned care team. Practitioner andRegistered Nurse staff may delegate assignments to the Care TeamSpecialist. CTS staff at GHC-SCW will have successfully completedmedical assistant training, licensed practical nurse training, Paramedictraining, or EMT training from an accredited program.
