Description:
POSITION SUMMARY: The Case Manager LMSW is a professional who collaborates with physicians,
health care team members and patient/families to coordinate care for specific patient populations across
the continuum and between care settings during an episode of illness. Provides various social work
services to meet needs of patients and family members. Services include psychosocial evaluation of
patient and relationship to family members in considering patient medical needs and prognosis. Provides
crisis intervention as necessary to patient and family members. Provides discharge planning for all
patients as necessary to ensure a smooth transition. Social work practices and services are consistent
with applicable accreditation and licensing standards, and reflect the National Association of Social
Worker Code of Ethics
POSITION QUALIFICATIONS
Masters degree in social work required. Prior employment experience in a related areapreferred.
Demonstrate ability and skill in the management of patient care planning and continuousquality improvement inservices delivered.
Recognized ability to be a key contributor as a member of a patient care team.
Well developed communication skills, both written and oral. Ability to communicate
effectively with individuals and groups representing diverse perspectives.
State of Michigan, social work license required.
Ability to work flexible work schedule.
Strict adherence to patient confidentiality expectations.
Knowledge of applicable accrediting and regulatory standards.
Articulates and demonstrates commitment to the philosophy, values, mission and vision
