THE OPPORTUNITY:
In collaboration with the Director of care transformation IL, the VicePresident, Care Transformation IL and BH, and MHN Chief Medical Officer,this position assists with the implementation on MHNs model of careacross the system. They provide accountability coaching to medical homesin achieving a common set of goals to control costs and ensure quality;creates a culture of provider engagement at all levels throughemployment of evidence-based practices.
WHAT YOU CAN LOOK FORWARD TO:
Serves as a leader member of the Care Transformation Team contributingto the goals and activities of the Care Transformation IL division.
Provides account management and oversight to MHN ACO REACH and MHN ACOprimary care practices; Assists Provider Champions, Care Managementleaders and other members at the medical home sites with the continuousimplementation of the MHN model of care. Evaluates practices againstperformance goals and provides direction for advancement.
Serves as the lead or co-lead of Care Transformation projects tied toMHN and/or Clinical Integration strategic goals; this spans initiationto completion and includes establishing the vision, project plan,implementation plan, communication plan and creation of evaluationactivities related to the project.
Acts as a subject matter expert on MHN clinical sub-committees and teamimprovement task forces.
Provides technical assistance to medical homes with MHNConnect 2.0portal and acts as a liaison for development needs. Provides UAT testingfor the platform and contributes to the technical requirements for newtechnology builds.
Supports management of health plan and other contracts in implementationof programming and reporting required, as well as supporting health planactivities needed to maintain compliance such as handling referrals,grievances, participating in interdisciplinary rounds, contributing toaudits, and articulating network needs.
Contributes to the formation and implementation of clinical andoperational initiatives using evidence-based practices to drive resultsacross the MHN population significantly improving care and reducingcosts.
Ability to supervise staff indirectly or directly.
Interpret and request needed data and reporting to provide caretransformation support and oversight. This includes the creation ofdashboards and other reporting to achieve benchmarks and KPIs.
Contribute to the development and oversight of Disease Managementprograms and other special initiatives.
Complete 1-3 on-site meeting at assigned medical homes per month, asable.
Contributes to the development of the full range of care managementtools, healt and wellness tools, and techniques including health riskassessments, artificial intelligence, screening tools, and patient andfamily educational and engagement tools; Assists sites with theirimplementation.
Develops and provides education and training of medical home staff,physician and providers; utilizes webinars and collaborative learningmodels that may be disease, patient experience or operationally focused.
Builds relationships with community agencies, care delivery andeducational partners/contractors; may serve as the liaison to theseentities.
