About LTE Case Manager I

  JOB REQUIREMENTS: The Dane County Human Services Department is currentlyhiring for an LTE Case Manager I in BHRC (Behavioral Health ResourceCenter). Hours of operation are 7:45am - 4:30pm, M-F (actual work hoursand days are TBD) and hybrid work environment. This position is locatedat 818 W Badger Rd, Madison. Limited Term Employees (LTEs) are limitedto 1,200 hours in a payroll year and are not guaranteed any hours perweek. DEFINITION CaseManagers work closely with children, adults, families, guardians,service providers and natural supports. Under the direction of theassigned Unit Supervisor, this position works with individuals in thecommunity to assess their psychosocial needs, determine eligibility forspecific services, and connect them with identified programs. Examplesof Duties Behavioral Health Resource Center (BHRC): Provide accurate andinformed referral information to Dane County residents seekingbehavioral health treatment. Work closely with community partners toidentify individuals who would benefit from assistance in accessingservices. Advocate for those who experience barriers to accessingbehavioral health treatment. Communicate with providers, insurers, andother third party professionals to assist in service delivery throughoutsystems. Make referrals for crisis services as necessary. Provide activeand back up phone consultation and drop-in support for BHRC consumers.Manage active caseload in collaboration with other team members.Document all calls, contacts, referrals and activities in database orrecord keeping system. Provide adequate follow-up services after makinga referral to verify referral completion or to problem-solve unforeseenbarriers to establishing and remaining engaged in meaningfulrelationships with behavioral health providers. Consult with other DaneCounty Department of Human Services (DCDHS) staff (child protection,developmental disabilities, long term care, etc.) regarding access toservice and service prioritization and authorizations. Assist withdevelopment and maintenance of BHRC website, behavioral health resourcedatabase, and other targeted resource materials. Perform other duties asassigned. ***** OTHER EXPERIENCE AND QUALIFICATIONS: Requires aBachelor\'s Degree in a human services field (social work, psychology,sociology, nursing, counseling, etc.) and the equivalent of one (1) yearof full-time experience working in a treatment setting with persons withmental illness and/or substance use disorders.
