$2,125 Gross Weekly
Under the supervision of a Registered Nurse, the LPN/LVN(Licensed Practical/Vocational Nurse) – Home Health provides skilled one-on-onenursing care to patients in their home and accompanies them when indicated toappointments, etc.
Essential Duties and Responsibilities:
Documents each shift or visit to reflect accurately theprovision of skilled nursing care rendered in accordance with physician’sorders.
Changes in patient condition are identified and documentedwith resolution on the appropriate clinical form. Director/Supervisor notification if applicable
Physician notification to obtain necessary orders forintervention(s) per state regulations
Performs specific treatments and medication administrationin accordance with physician orders and home health agency’s protocols
Documentation reflects patient collaboration andparticipation in their plan of care
Requests for modification of orders and additional ordersare made based on the patient’s clinical status and skilled nursing judgment
All physician collaboration/coordination of care isdocumented in the medical record and communicated as needed
Provides patient and family education as appropriate
Attends case conferences, team meetings and educationin-services as required
Minimum Requirements:
Graduate of an accredited practical or vocational nursingprogram
Currently licensed as a Licensed Practical/Vocational Nursein the state in which the LPN/LVN will practice
One year prior professional nursing experience, home careexperience preferred
Complies with all relevant professional standards ofpractice
Participation and completion of Maxim's Competency programwhen applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
Computer proficiency required
Must be at least 18 years of age
