JOB REQUIREMENTS: The LPN: Licensed Practical Nurse supports our missionand values by providing compassionate and holistic care for theresidents based on individual care plans. What will I do as an LPN?Safely administer medications and treatments as ordered by the physicianand in accordance with accepted standards of practice and Three Pillarspolicies and procedures. Maintain timely, accurate written andelectronic charts/documents/records as required by law, regulations andThree Pillars policies and procedures. Observe and record residentcondition information. Report findings to and collaborate with RN todevelop and implement plan of care for the resident. Communicateresident change of condition to appropriate persons. (Nursemanager/supervisor, RN, physician, family, clergy, hospice team,interdisciplinary team, etc.) Assist in the admission/readmissionprocess as directed by the RN including observing and gathering ofclinical data and development and implementation of the initial plan ofcare. Obtain, transcribe and implement physician\'s orders according toaccepted standards of practice and Three Pillars policies andprocedures. Support and participate in all resident care programs (ieBowel and Bladder, Skin Integrity, Pain, etc.) including followingindividual POCs, documentation, communication of observations, andoffering of feed back/suggestions.
