　　Job Overview

　　In response to collected data, implements, and evaluates nursing caredesigned to meet the individual health and illness needs of complex patientsand their families in collaboration with other disciplines. Carries outpatient responsibilities under direct supervision of registered nurse withemphasis on family-centered care.

　　Department: Gastro/Liver ClinicHours per week: 36, eligible for benefitsShift: Monday-Friday (one set day off each week). No weekends,holidays or nights.

　　Qualifications

　　Education: High School Diploma or GED and completion of a Practical Nursingcertification or diploma program.Experience: One (1) year of LPN experience.Certification(s): BLS from the American Heart Association / CPR.Licensure(s): State of Colorado Department of Regulatory Agencies(DORA) Licensed Practical Nurse (LPN).

Reports signsand symptoms of deviation from expected health or plan of care to RN orProvider for full assessment.Provides routine and common education to patients and familiesUnder the supervision of the RN, provides developmentally appropriatecare, interventions and evaluates the effectiveness of interventions andcommunicate outcomes of care given.Develops goals to advance clinical practice and professional growth thr ugh anunderstanding of the need for self-directed personal and unit basedprofessional and educational activities.Participates in quality and process improvement initiatives.Understands the roles within the multidisciplinary team providing familycentered patient care while maintaining objective and professional boundaries.
