　　We are seeking a full time MSW-LCSW Social Worker to join our Development and Behavioral Health team of professionals. We provide outpatient services, Monday-Friday.

　　Job Description: Under direct supervision, MSW-LCSW will coordinate the treatment program, serve as liaison between the treatment team, patient, family and payer source in moving the patient through the treatment program, provide case management services including obtaining authorizations for treatment and additional necessary case management duties, coordinating, facilitating and attending family conferences, identifying and referring to community resources, and providing counseling support as needed.

　　Education: Master’s degree in social work from a school accredited by the Council on Social Work education.

　　Experience: A minimum of 1 year experience in medical case management desired.

　　Kansas/Missouri state licensure or accreditation in field is preferred. Evidence of training or experience in medical case management as evidenced in resume work and training history.
