　　Job Summary:

　　As a Social Worker at Trinitas Regional Medical Center RWJ Barnabas Health, you will provide frontline services to patients with conditions spanning the entire health care continuum.

　　Understands financial responsibilities of the case management department and safeguards system resources by actively participating in utilization management and insurance communication

　　Collaborates closely with Physicians, APN, CNM, and Nursing staff

　　Participate in conferences with the Women's Health Center (WHC) and Pediatric Health Center (PHC) team to ensure good patient outcomes

　　Complete psychosocial assessments and implement patient-centered care plans that meet needs identified in assessments

　　Discharge planning activities seamless and timely coordination of care including referrals to community resources agencies to assist with pediatric prenatal and postpartum care and transportation arrangements

　　Collaborates with inpatient MCH LSW and Director of case management to provide a seamless transition from outpatient to inpatient for mother and baby

　　Collaborate with department leadership to streamline processes and implement strategies to create more efficient delivery of care

　　Ensures compliance with regulatory documentation standards

　　Job Requirements:

　　Graduation from an accredited graduate school of Social Work with a MSW Masters in Social Work degree

　　State licensure required by the NJ State Board of Social Work Examiners

　　Licensed Social Worker LSW or Licensed Clinical Social Worker LCSW depending upon years of social work experience and clinical course work

　　Successful completion of all orientation programs
