Pay: $27-$32 per hour based on experience. (18% increase in hourly pay with PIL program)

Schedule: Monday-Friday, 8a-5p

Graduate of a school of professional nursing required.Cardio Pulmonary Resuscitation (CPR) certification requiredminimum of one year's experience in working with the frail or elderly population requiredMust have a valid driver's license, proof of insurance and have means of transportationMust have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contactIntravenous Venous (IV) Certification preferredEssentials:

Uses and practices basic assessment and assists Clinic Supervisor as designated to complete initial assessment of new PACE enrollee.Functions as a member of the Interdisciplinary Team. Maintains regular attendance at and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.Uses and practices basic assessment and assists Clinic Supervisor as designated to complete annual reassessment.Performs basic nursing services as delegated by Clinic Supervisor.Performs the administration of prescribed medications and treatments in accordance with nursing standards.Maintains participants' medical record and fulfills Senior Community Care charting and reporting requirements.Observes, records, and reports participant's condition and reaction to drugs and treatments to physicians.Senior Community Care of Kentucky - PACE:
