  Minimum Qualifications MINIMUM QUALIFICATIONS:Candidates must be reachable for appointment from the current Civil Service eligible list to be considered for a permanent competitive appointment. Licensed Master Social Worker 2 – Licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW) and have one year of post NYS licensure professional social work experience; * OR Licensed and currently registered by New York State as a Licensed Clinical Social Worker (LCSW). Registration and licensure as a Licensed Mental Health Counselor are not considered qualifying. * Professional social work experience involves activities such as screening, assessing and diagnosing individuals' and families' psychosocial needs, strengths and limitations; participating in the development of treatment plans and discharge plans; counseling and educating individuals, groups and families to promote mental, emotional, physical and social functioning; helping access needed services; providing client/patient advocacy and crisis intervention; and developing, coordinating and evaluating services and support systems. Experience limited to social work research or social work administration is NOT considered qualifying. Continued employment as a Licensed Master Social Worker 1 or 2 is dependent upon current licensure and registration by New York State as a Licensed Master Social Worker or Licensed Clinical Social Worker. Loss of licensure or registration may result in removal from employment.

  Duties Description POSITION DESCRIPTION:This position will serve as a Licensed Master Social Worker for a division within the Developmental Disabilities Regional Office 2. Divisions within the Regional Office include the following: Service Access (Front Door and Waiver Services), Eligibility, Self-Direction, Housing, Crisis, Property/Development, Fiscal, and Quality Management/Technical Assistance. Regional Office 2 supports over 1,800 individuals with developmental disabilities who reside in 20 counties of NYS: Broome, Cayuga, Chenango, Clinton, Cortland, Delaware, Essex, Franklin, Hamilton, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, Otsego, St. Lawrence, Tioga, and Tompkins. #LI-AR1Supports are provided via a network of approximately 110 agencies who are the direct providers of services. Travel to the three districts within Region 2 may be required, as well as intermittent travel to other areas of the state.PRIMARY DUTIES AND RESPONSIBILITIES:• Assist individuals with disabilities, together with their families/advocates through all of the steps of the front door key process steps meeting targeted timeframes.• Work in partnership with front door team members under the supervision of the front door field leads and department leadership. • Provide individuals/families with information/education on opwdd eligibility process, service options, referral and advocacy. • Provide information and connect individuals in the front door process to care coordination organizations or service access agencies • Develop a working relationship with care coordination organizations staff. • Initiate, maintain, and conclude the choices eligibility, assessment and authorization record. • Complete needs assessments (currently the developmental disabilities profile 2) (ddp2). • Review the coordinated assessment system/child adolescent needs and strengths and other supporting documentation as part of a comprehensive evaluation/ determination of the applicants service needs.• Review and process level of care eligibility determinations. • Confirm Medicaid status/enrollment; communicate with the financial benefits and entitlements assistance and management office to ensure individual is enrolled in the correct plan needed for supports and services. Facilitate parental deeming process when determined appropriate. • Triage service enrollment needs and ensure that necessary documentation is obtained and reviewed. • Present service package to the quality review team and complete service authorization process. • Engage in self audit and ongoing monitoring of caseload. • Work in choices and tabs.• Maintain spreadsheets and data regarding front door process and caseload management. • Working knowledge of SharePoint and one drive.• Participate in the ongoing development of office process and procedures. • Occasional overnight travel to support attendance at trainings, meetings, conferences. At times these could occur in other areas of the state and may require overnight travel.
