POSITION SUMMARY
Provides direct behavioral health and case management interventions toindividuals involved in the criminal justice system, assisting individualsto gain needed resources and services and facilitating their engagement intreatment. The position also provides outpatient competency education toindividuals referred by the Colorado Office of Civil and Forensic MentalHealth.
CORE RESPONSIBILITIES
Provide supportive behavioral health interventions to clients either inindividual or group settings, including MRT GroupsComplete Brief Screening of all clients referred through a judicial entity(Probation, Parole, Court, Restoration Program, Youth Diversion andDUI) and engage client in case management services as appropriateDevelop, monitor, and follow up on client case management service plansAssist clients with resource acquisition, linkage to agency and communityservices, behavioral health skill developmentWorking collaboratively with the treatment provider(s), coordinate (beknowledgeable about) services provided to judicially referredclients. Examples include: communicating weekly with therapist(s) on allnew judicially-referred admissions; making sure that CM services areincluded in the Treatment Plan for all clients served, attend clinical teamor ACT meetings; confirm client attendance and UA results with clinician ofrecord, work with clinician to enhance treatment engagement and success,etc. enter client data in TMS web-portal (for DUI clients if assigned).Serve as primary source of contact and information for referral parties,including local probation departments, courts and others; attend probationmeetings regularlyIf assigned, represent the agency in local problem-solving courtsProvide services in a variety of settings, including client homes,community locations and Centennial officesMaintain accurate and timely clinical records consistent with Center standardsParticipate in efforts to collaborate with local jails to facilitate carecoordination for individuals releasing from incarceration into CentennialcommunitiesServe as a Case Management resource to individuals releasing from localjails, assisting with engagement into treatment and other practical needsduring the transition phaseIf assigned, facilitate the DRS process for DUI clients by entering TMS dataregularly and providing the completed DRS form to clients when treatment issuccessfully completed.Respond to referrals for Outpatient Competency Restoration services in atimely mannerProvide outpatient Restoration services to referred clients as directed byOffice of Civil and Forensic Mental Health, and complete all requireddocumentation and reporting relating to these services if assignedParticipate in meetings, training and in servicesParticipate in supervision sessionsPerform other job duties as assignedDemonstrate a commitment to continuous skill development to competently serveindividuals from diverse cultures and backgrounds.
RequirementsEDUCATION
Bachelor's degree in psychology, social work, or other human servicesfield required
Preferred Experience
Experience working in behavioral health, human services, adulteducation, rehabilitation preferred but not required
