  The Interventionist provides 24/7 real-time access in the form of family-based interventions, patient needs alignment, and resource access via phone or in home visit, local community, or clinic.

  Essential Functions

  Employees must abide by all Joint Commission Requirements including but not limited to sensitivity to cultural diversity, patient care, patient rights and ethical treatment, safety and security of physical environments, emergency management, teamwork, respect for others, participation in ongoing education and training, communication and adherence to safety and quality programs, sustaining compliance with National Patient Safety Goals, and licensure and health screenings.

  Must perform all duties and responsibilities in accordance with the hospital’s policies and procedures, including its Service Standards and its Code of Conduct.

  Departmental functions include:

  Conduct a comprehensive assessment upon enrollment of a new patient, including but not limited to review of referral information; engagement of key people in the child’s life, including his/her family, Stanford healthcare providers, and community agencies; identification of strengths and weaknesses; and synthesis of all relevant information into an analysis of the patient within his/her ecological context.

  Collaborate with the NICH Clinical Team to develop direct clinical treatment (e.g. strategic family and systems interventions) compatible with NICH principles and practices.

  Implement problem conceptualization, treatment plan, intervention implementation, outcome review, and strategy revision procedure under the supervision and direction of a licensed clinical psychology supervisor.

  Liaise with the patient's healthcare team and assist the family with navigating the health system.

  Collaborate with key participants within relevant systems to ensure buy-in and cooperation throughout NICH treatment. Integrate the health systems involved in the patient’s care.

  Facilitate resource access and patient optimization that directly relate to the patient’s medical care.

  Maintain clear and concise documentation of treatment efforts.

  Participate in all model program training, supervision, and consultation activities.

  Strong passion for improving the quality of life of youth with chronic health conditions and their families.

  Provide 24/7 real-time access in the form of family-based interventions, patient needs alignment, and resource access. This can occur via phone or in the home, local community, or clinic, all within a 40-hour work week with flexible timing including evenings and weekends depending on family availability.

  Minimum Qualifications

  Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

  Education: Bachelor's degree in public health, mental health or human development related from an accredited college or university or equivalent combination of training and experience.

  Experience: Two (2) years of progressively responsible and directly related work experience working with youth, individuals with chronic illness, community programs, and/or coaching may also qualify at the discretion of the agency.

  Knowledge, Skills, & Abilities

  Ability to communicate effectively, both orally and in writing

  Ability to plan, organize, prioritize, work independently and meet deadlines

  Ability to work effectively with individuals at all levels of the organization

  Knowledge of computer systems and software used in functional area

  Pay Range

  Compensation is based on the level and requirements of the role.

  Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data.

  Typically, new team members join at the minimum to mid salary range.

  Minimum to Midpoint Range (1.0 FTE): $67,433.60 to $76,492.00
