This role requires direct interaction with patients, family, and referral sources. Customer service and attention to detail are critical to the success of this role.ResponsibilitiesEnsures that all patient registration forms are complete, and coordinate with Regional Administrator if key pieces of information are missing. Including consent for treatment, problem, and medication historyProvides education to inquiring patients or referral sources about ACP services and benefitsCreates new patient accounts, and data enters pertinent patient medical history, and contactsGather information and documents from patients, HIE feeds, or other EMR systemsBuild relationships by providing friendly, timely responses about patient registration process, and set expectations of next stepsProcess incoming requests for records from patient/caregiver, payors, or other health care professionalProcess provider requests for records, such as previous PCP, specialist, lab & diagnostic results, hospital or LTC recordsEnsuring that the medical records are organized, accurate and completeCommunicates with other departments regarding the status of referralsMaintains confidentiality of patient and proprietary information. Ensure compliance with ACP medical records policy, in accordance HIPAA policyQualificationsHigh School Diploma/GED or equivalent; Associate degree or some college preferredMinimum two (2) years of experience collecting referral information in the healthcare marketMedical Assistant certification preferred, but not requiredExperience working with all payer types, including Medicare, Medicaid and commercial insurance companiesUnderstands the scope of services that Abode Care Partners and BrightSpring sister companies will offerStrong verbal and written communication skillsAbout our Line of BusinessAbode Care Partners is a leading provider of integrated medical services caring for individuals from post hospitalization to home in various settings ranging from skilled nursing facilities, assisted living, independent living, group homes, and private homes.
