About Intake Coordinator

  The Intake/Clinical Coordinator is responsible for maintaining the files and records for the children placed in New Horizons Residential Treatment Center. This includes filing, typing, dictation, mailings, logging, and participation in various clinical meetings.

  All New Horizons Staff are expected to study, practice, and operate within the New Horizons’ Philosophy of Keeping Kids First. Keeping Kids First requires that every decision and action be motivated by and geared toward meeting the highest need of the individual kid. New Horizons will not tolerate anyone who is not committed to keeping kids safe from abuse, neglect and exploitation.

  Duties and Responsibilities

  Attend staff meetings and other programs that ensure continuing education to meet the annual requirements.

  Screen intake calls and packets; Coordinate intake packets with Placement director, Administrator, therapists and case managers; Communicate with placement coordinators regarding placement consideration of children.

  Coordinate and schedule intake interview with placement personnel/guardians and therapist, conduct interview by gathering information and signatures for admission paperwork, and help transition student into residential care by communicating with staff.

  Complete Preliminary Service (72 hour) Plan within appropriate timeframe and provide copies to treatment team members working with that child.

  Complete Admission Assessment. Ensure therapists have provide Goals and Objectives.

  Create the initial student files and enter all data of placed students into Extended Reach. 

  Update new Level of Care in database when needed.

  Perform other duties as assigned by the Placement Director.

  Be a part of the weekly clinical meeting and provide participants in the meeting with information such as current census, and any upcoming admissions and discharges.

  Qualifications

  Must be 21 years of age and be a high school graduate or equivalent, (GED).

  Must have a minimum of two to three years of experience in the social work field.

  Applicants must pass criminal background checks, a drug screening, and a tuberculosis test.

  Must have a valid drivers license. 

  Must be able to perform conflict resolution or crisis intervention

  Must possess positive verbal and written communication skills, the ability to make logical decisions in times of crisis and work well in a team and independently. 

  Must demonstrate effective oral and written communication skills.

   Must demonstrate effective organizational skills.

   Must have good time management skills and excellent public relation skills.

   

  Working Conditions

  This position reports to the Placement Director.

  Frequent day travel is required but will be reimbursed at the going rate.  Must be flexible to work after hours or evenings, occasionally for emergency placements.

  This position is expected to properly communicate while independently making decisions that keep kids safe and while assuring that all standards and requirements are met.

  This position will interact with many different persons and families in varying stages of crisis.

  Audrey Grace House is a residential treatment center for children, ages 5 - 17, suffering from emotional, behavioral, and attachment or relationship issues. The home accommodates 40 children and length of stay usually lasts 9 to 12 months.
