If you're motivated to give our clients a more positive quality of life we encourage you to apply today!ResponsibilitiesGuides and counsels individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for themAssists clients in obtaining government funds, education, or treatmentSecures information, such as medical, psychological, and social factors contributing to client's situation, and evaluates these and client's capacities Asks clients to explain problems, such as personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine nature and degree of problemHelps patient and family through individual or group conferences regarding plans for meeting needs, and aids client to mobilize inner capacities and environmental resources to improve social functioningHelps client to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and client's part in creating themDevelops service plan designed to restore patient to optimum social and health adjustment within patient's capacityRefers clients to community resources and other organizations, and liaises with other social service agencies. Utilizes community resources to assist patient to resume life in community or to learn to live within limits of disabilityPrepares patient histories, service plans, and reports.
