Responsibilities
Responsible for providing home infusion therapy services to assigned patients per medical provider orders, utilizing professional skills to develop and administer a plan of care that meets the patient’s specific needs and is in compliance with state and federal regulations, as well as URMHC and UR Medicine Pharmacy policies, procedures and medication protocols.
Participates in the orientation of new Home Infusion RNs by acting as preceptor, and provides ongoing education and mentoring of team members to ensure the team’s clinical competence, accuracy of documentation and appropriate RL solutions documentation and resolution of customer complaints
Participates in monthly quality audits in conjunction with LHCSA Director
Collaborates with LHCSA Director in the annual review process
Works in partnership with UR Medicine Pharmacy and LHCSA Director to assure delivery of high-quality patient service and clinical outcomes.

Qualifications
Completion of an accredited Registered Professional Nursing Program, Bachelor’s degree preferred
Two years’ experience as a Registered Professional Nurse, including at least one year providing home health care services in a certified, licensed or hospital approved agency.
One-year minimum Home Infusion experience required.
Licensure, Certification, and/or Credential Requirements
Current New York State RN License in good standing
Current BLS CPR Certification
Certified Registered Nurse Infusion (CRNI) Certification preferred
Valid NY State Driver’s License and reliable transportation
Knowledge, Skill, & Ability Requirements
Ability to work with patients, families, and staff from diverse backgrounds
Outstanding interpersonal and communication skills, both verbal and written
Excellent problem solving and decision making skills
Self-directing and assumes responsibility for own continuing education using formal and informal resources that contribute to professional self-growth

Salary Range: $89,608 to $97,428
