About Home Health Aide/ Personal Care Aide

　　· Ensure quality and safe delivery of home health care services.· Implement client’s plan of care in accordance with the Agency’s process and procedures policy.· Participate in Client’s case conferences according to the Agency’s policies and regulations.· Submit updates and relevant information regarding the Client’s plan of care to the Registered Nurse in a timely manner. · HHA/PCA will work with patients of all ages that require care in their homes.· HHA/PCA must be able to carry out patient care plan activities.· HHA/PCA must be caring, patient and compassionate caregiver.· HHA/PCA must be able to accompany patients to doctor’s offices or to hospital visits.

　　Minimum Salary: 19.00Maximum Salary: 20.00Salary Unit: Hourly
