This is an Advanced Practice Professional (APP) job opening, hiring Nurse Practitioner(s) and/or Physician Assistant(s) who will see same day appointments in a primary clinic setting. Both qualified/experienced NP and PA applicants will be considered.
This position will be located at HF Detroit, Fairlane's location.
Advanced Practice Professional will provide health care and specialized health services to patients and families enrolled in Primary Care Medicine. The APP will coordinate care of this patient population in collaboration with all members of the health care team. This position will be 100% outpatient. APP will provide outpatient medical care by taking patient history, performing physical examinations, orders, and interpreting tests, making diagnoses, and institutes appropriate treatment programs.
Ideal candidates will have 1-2 years’ experience as an APP.
Independently evaluates patients in the clinic.
Ordering/interpreting testing and lab results, modifying plan of care to address results
Provide clinical expertise and direction to support patient medication management
Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external team members and other ambulatory care providers
Perform other related duties as required to assure patient safety and satisfaction
Participate in the development and presentation of educational experiences for patients, families, and health care professionals
Procedures– specific procedures are individually credentialed as indicated by training and experience
Schedule– Monday – Friday, days
Weekend requirement– Yes, 1 weekend morning a month

GENERAL SUMMARY: Provides inpatient and/or outpatient medical care. Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently. Specific procedures are individually credentialed as indicated by training and experience. EDUCATION/EXPERIENCE REQUIRED: Master's degree in nursing. CERTIFICATIONS/LICENSURES REQUIRED: Certified Nurse Practitioner. Licensed as a Registered Nurse. Current BLS or ACLS as required. Current ATLS/PALS provider status as required by department.
