  About the Work Unit:

  This work unit exists to direct and coordinate services and supports within the Intermediate Care Facility for intellectual and developmental disabilities (ICF-IDD) program to assure individuals goals promote independence to the fullest potential are developed and achieved and is responsible for ensuring staff implement the Individual Program Plan in accordance with State and Federal regulations.

  Description of Job

  About the Position:

  This position exists to plan, develop, and coordinate comprehensive services provided to individuals with intellectual disabilities and safeguard and advocate for their rights, safety, and quality of life. This includes coordinating administrative functions and services for the resident. This position ensures the implementation and documentation of residential programs is completed. This position is responsible as a supervisor for two to three Health Care Technician IV’s, one Community Integration Coordinator (HPII) and 20-35 direct care staff as a second level supervisor. This position also works in coverage on the homes as needed. Duties in this position include:Appointing Authority and Supervision:

  exercise appointing authority responsibilities with first and second level supervision over Residential and Community Integration staff, including approximately 30 employees

  directly supervise four Health Care Tech IV positions including oversight of hiring, performance planning, performance evaluation, corrective, and disciplinary action, up to and including termination

  provide directive for daily operations and processes including identification of necessary training. Community Integration staff along with residential living site staff will follow this supervisor

  as 2nd level supervisor, collaborate with, coach, and advise subordinate Health Care Tech IVs who each supervise up to 11 employees, including performance planning, performance management, and disciplinary actions.

  directs the overall operations of each of the living area or department, including approving the allocated budget, consistency of policy & procedures, and strategy for each

  ensures regulatory compliance of all agency departments in conjunction with: DPA regulations; Personnel Rules; State Board of Nursing Practice Acts; CDHS Policies and Procedures; and other state and federal mandates

  Administrative & Duty Officer:

  directs, oversees, and monitors delivery of comprehensive services to individuals by facilitating or co-facilitating the Interdisciplinary (ID) Team (medical, programming, psychology, nursing, OT, PT, nursing, clinical behavioral specialists) process to develop an Individual Program Plan (IP) by reviewing the individual’s progress in reaching identified goals in the IP and by recommending ways to overcome barriers to reaching goals

  responsible for routine monitoring, including conducting regular audits of each Individual’s program in Therapy

  alerts supervisors of any issues arising with program implementation or the documentation thereof. This is a Federal requirement of ICF-IDD licensure

  understand the importance and necessity of disciplinary action to support the success or validate the failure of an employee

  serves as agency Duty Officer, and/or as the Crisis on Call, for one week duration assignments

  responsible for making administrative decisions and intervention decisions for people receiving supports and services as well as staff

  ensures regulatory compliance of all agency departments in conjunction with: State Board of Nursing Practice Acts; Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF-IDD) and Home and Community Based Services (HCBS); Health Facilities and Emergency Medical Services Division (HFEMSD) of

  Department of Public Health (DPH); Centers for Medicare and Medicaid (CMS); CDHS Policies and Procedures; and other state and federal mandates

  Regulatory Compliance:

  assures the effectiveness of the Interdisciplinary Team (medical, programming, psychology, nursing, OT, PT, nursing, clinical behavioral specialists) process by identifying, coordinating, and providing leadership, guidance and training to team members (professional and para-professional) when appropriate in various subject matters related to ICF-IDD & Waiver-HCBS

  identifies and resolves problems of the individuals we support by directing and assigning specific tasks to ID team members for problem resolution.

  assures active treatment components are in place and implemented for each individual we support

  provides Case Management services to the individuals we support

  Provides Supports, Services and Other Duties as Assigned:

  respond to behavioral challenges as needed with the implementation of specific Behavior intervention programs (including Safety Control or Emergency Control)

  must be annually certified for the skilled implementation of Safety Care. This position may be required to work other locations and/or shifts as needed to address agency resource needs. This position must have or get QMAP trained or have an LPT license

  this position will complete other duties as assigned by their supervisor or agency director.

  Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

  Minimum Qualifications

  Bachelor's degree from an accredited institution in an academic discipline associated with the study of human behavior, human skill development, humans and their cultural behavior, or any other services related to basic human care needs or the human condition.

  AND

  Four (4) years of professional experience that must include experience with IDD and dually diagnosed individuals and progressive management

  Substitutions

  Additional education will substitute for the experience on a year-for-year basis.

  Special Qualifications:

  Due to federal requirements for ICF-IID positions, work experience may not be

  substituted for the bachelors degree.

  Preferred Qualifications:

  Preferred candidate will have one (1) year of supervisory experience

  demonstrated knowledge of the Performance Management Plan (PMP) process

  demonstrated ability to communicate effectively

  demonstrated skills prioritizing and organizing work

  demonstrated ability to interpret rules and regulations

  advanced skills in using Google applications and general computer use

  Conditions of Employment:

  Must have valid unrestricted Colorado Driver License

  A pre-employment criminal background check will be conducted as part of the selection process as this position is deemed to have direct contact with vulnerable persons. Felony convictions, conviction of crimes of moral turpitude, or convictions of misdemeanors related to job duties may disqualify you from being considered for this position. Should your background check reveal any charges or convictions, it is your responsibility to provide the CDHS Background Unit with an official disposition of any charges.

  On Call Status- Serves as Duty Officer to consult with staff after hours

  Position must travel to remote work sites and must utilize state vehicles, if available. If not, must utilize personal vehicle and submit for mileage reimbursement.

  Former employees of the Colorado Department of Human Services (CDHS) who were disciplinary terminated or resigned in lieu of termination must disclose the information on your application and may not be eligible for employment at CDHS.

  Influenza vaccination required

  On or after October 31, 2021, each facility shall ensure all newly hired employees or support staff members are fully vaccinated at the time of hire or receive their first dose of a two-dose protocol, or a single-dose vaccination, prior to providing any care, treatment, or other services for the facility and/or its clients. Upon hire, new employees will have three (3) business days to provide attestation to their status with proof of vaccination. Vaccinated employees must provide proof of first dose prior to providing care. Note: Fully Vaccinated means two (2) weeks after a second dose in a two-dose series of the COVID-19 vaccine, such as the Pfizer, Moderna or Novavax vaccine, or two (2) weeks after the single-dose vaccine, such as Johnson & Johnson’s Janssen vaccine.
