Description
FUNCTION:Under the supervision on the Division Director, the Specialist will enhance its current funded FSS (Family
Supports and Services) programs including Parenting Program Group to serve more people with developmental disabilities
and provide additional in-home and one-to-one parenting training sessions. The mission is to provide person-centered
services to people with developmental disabilities to strengthen the family unit and avoid out-of-home placement of their
children or termination of parental rights.
RESPONSIBILITIES:
1.Recruit participants, via ULR Programs, Community outreach, CCOs, Courts, Schools, and the Child Protective Systems
Submits completed applications for individuals with developmental disabilities seeking financial reimbursement for goods and services such as adaptive equipment and medication that enhance the quality of life for individuals who reside with a family.
Completes monthly request for payments to reimburse approved goods and services.
Coordinate and Chair, once a month, FSS committee meeting to review all pending applications for FSS.
5.Support weekly support group classes alongside parenting teacher.
6.Provide one-on-one parenting support to a caseload of 20 parents.
7.Administer Program on daily basisbyproviding case management services to custodial & non-custodial parents.
8.Assist with administrative duties as needed.
9.Maintain a minimum of twice per month contact with each individual on assigned caseload.
10.Link & refer individuals to needed services within Urban League or in the community.
11.Complete records & reports to track & assure successful program outcomes in a timely manner.
12.Participate in training and orientation as required and approved by Division Manager for effective program operation.
Attend all Urban League of Rochester events and fund-raising functions.
Perform other duties as the supervisor may assign.
Requirements
QUALIFICATIONS:
· Associates Degree in related filed of Human Services
· Must have a Minimum of 2 years’ experience working with Individuals with Developmental Disabilities.
· Must be knowledgeable of life plans, monthly summaries, staff action plans and Medicaid billing.
· Must be technological proficient using case management software i.e. (Precision Care, eVero, Connections)
· Must be familiar with Zoom, WebEx, Microsoft Teams or other virtual platforms.
· Must be proficient in Microsoft Office: Word, Excel, PowerPoint, and Outlook
· Familiar with OPWDD regulations
· Ability to relate well with others, a team player.
· Must provide own transportation. Valid New York State Driver’s License.
· Ability to work evenings when required.
Salary Description
$16.50 - $18.00 an hour
