Participate in referral relations activities within an inpatienthealthcare setting and conduct preadmission patient evaluations.Coordinate the referral process to ensure a smooth transition ofpatients to post-acute services within Mass General Brigham (MGB) scopeof care. Obtain patient assessment information from a variety of sourcesand transmit data to appropriate systems. Develop and maintaincollaborative relationships with internal staff and external referralsources. Determine appropriate discharge plan, communicate with referralsources, and resolve customer concerns with positive outcomes. Patientsare evaluated, interviewed, and referred to appropriate post-acuteservice within the MGB scope of care according to established guidelinesand in compliance with local, state, and federal regulations, includingLTAC, IRF, and SNF. Perform on-site (typically in referring facility)clinical judgement to determine the appropriateness of patient foradmission. Patient assessment information is obtained from a variety ofsources and data is transmitted in appropriate systems (4Next). Patientevaluations include meetings and interviews with patient, families, orother caregivers as appropriate. Collaborative relationships aredeveloped and maintained with internal staff to ensure customersatisfaction. Referral source relationships are nurtured to ensure andmaintain customer satisfaction. Challenging transfers are supportedusing MGB clinical and supervisory resources. Patients are accepted,confirmed, and qualified as described in the liaison process. Admissionsadhere to MGB criteria. Pertinent information from MGB is communicatedto the current medical provider to meet patient and provider needs. MGBreferral influences are identified and communicated to the manager forproper follow up per established guidelines. Discharge plan alternativesare considered to determine the appropriate plan to meet the patients?needs. Payor mix results are achieved according to established targets.Account/territory referral and admissions targets are achieved.Educational information and updates are shared with the referral sourcesto ensure an understanding of the benefits of MGB post-acute services.Additional department, organization, or network activities are completedper established objectives. EDUCATION/DEGREE High School Graduate or equivalent requiredEXPERIENCE Minimum 2 years of acute hospital, or extended care facilityexperience required. LICENSURE Current license in Massachusetts as RN,Physical Therapist, Occupational Therapist, Speech Language Pathologist.Skills (Specific learned activity gained through training (e.g. computerskills, keyboarding, presentation, CPR, ACLS, etc.) REQUIRED: Knowledgeof switchboard practices, paging procedures and equipment. Computerproficiency required. Microsoft office applications preferred withability to learn new software. Effective verbal, electronic and writtencommunications skills using the English language. PREFERRED: Broadknowledge of various insurances and government reimbursements isdesirable. Sales and/or marketing experience preferred. Highly developedcommunications and interpersonal skills, working with diversepopulation. Work independently, be self-directed and contribute as amember of a team.
