The Eligibility & On-Boarding Specialist, the Eligibility Specialistsupports and assists with enrollment, Medicare/Medicaid eligibility, andinitial and ongoing eligibility support for outreach services (SNAP,LEAP, housing, resourcing, etc.).
Required Qualifications: To perform this job successfully, an individualmust be able to perform each essential duty satisfactorily. The requirementslisted below are representative of the knowledge, skill and/or abilityrequired. Reasonable accommodations may be made to enable individuals withdisabilities to perform the essential functions.
Bachelor's Degree in human services or related field or three (3) yearsexperience coordinating senior services.A minimum of one year's experience working with frail or elderly populationrequired.Experience working with community-based housing, assisted living andlong-term care is preferable.Must have medical clearance for communicable diseases and up-to-dateimmunizations before having direct participant contact.Must have a valid driver's license, proof of insurance and have means oftransportation.Excellent customer-service orientation.
Essentials:
Facilitates and assists potential enrollees through county eligibilityprocess, and enrollment determination process, and enrollment into theprogram.
Communicates with the county in regard to eligibility, participant contactinformation, disenrollment's and any other necessary participationinformation including, but not limited to, enrollment, disenrollment,death, and permanent placement in nursing or assisted living facilities.
Monitors Medicaid and SNAP re-certification and partners with the county toensure timely recertifications and/or resolution of eligibility issues.Tracks and submits Functional Recertification with the county to ensureparticipant eligibility.
Upon permanent placement in a nursing or assisted living facility, informsparticipants and/or families of patient payment amount and completespaperwork for business office and completes a PETI or Supportive HousingAgreement.
Completes disenrollment packets and exit surveys for participants who aredisenrolling from the PACE.Assists in establishing our presence in the community by outreachingvirtually, telephonically and in-person.
Supports event participation, including set up booth/table, collateralmaterials, engagement interactions, and other supplies for exhibits andmeetings with community.
