SUMMARY:
Home Visitor at NEKCA supports families in goal setting, childdevelopment, and wholistic family well-being. Recognizing that familiesare their childs first teachers, NEKCA utilizes a strengths-basedapproach to help families thrive. Home Visitors utilize a variety ofstrategies, including home visiting and offering enrichment activitieslike community playgroups. Home Visitors are part of a larger EarlyChildhood Team at NEKCA that collaborates to provide seamless servicesthat put families at the center.
ESSENTIAL FUNCTIONS:
Carry out duties in compliance with HS Performance Standards,Content Area Plans, Vermont Child Care Licensing Regulations forCenter-Based Child Care and Preschool Programs, NAEYC, VermontParent Child Center Quality Standards, and NEKCA Standard ofConduct.Complete NEKCA Performance Evaluations, Professional DevelopmentSelf-Assessment, and in partnership with supervisor, IndividualizedProfessional Development Plan
Be aware of indicators of child abuse and neglect and comply withVermont Mandated Reporter Law and program Child Protection ProtocolParticipate in center staff meetings, monthly content area meetings,pre-service, and in-services and incorporate relevant knowledge andskills into practiceDUTIES AND RESPONSIBILITIES:
Maintain accurate, up-to-date files on all enrolled childrenOffer weekly home visits to enrolled families that last at least 1.5hrs. and provide a minimum of 46 visits per year (make-up visits arean hour)Plan 22 playgroup opportunities in partnership withparents/guardians enrolled in EHS and offer over the course of theyearProvide services to expectant families according to the programperformance standards and curriculumProvide orientation to the program with review of the FamilyPartnership Handbook, the Guide to Community Resources and with thecompletion of the Parent Engagement Survey and Family InterestChecklistProvide a monthly newsletter to all enrolled familiesAssist parents/guardians in co-planning for home visits, childdevelopment activities and playgroups per parent/guardian and childinterests and goalsPromote regular attendance in EHS activities and documentFollow program protocol in documenting home visits, missed homevisits, offer make-up visits, playgroup and parent attendance forall program offerings and all family contactsWrite a minimum of one observation in each developmental domain perchild per month for home visits and playgroupsPut no less than 9 observations per child per assessment period inTS GOLDConduct annual developmental and behavioral screenings inpartnership with parent/guardian within 45 days of enrollment andreview results with parent/guardian according to program protocolContact Early Head Start Children's Services Manager if screeningscore shows a concernImplement the EHS Curriculum to fidelityComplete DRDP Assessment 4 times a year according to timelineIn partnership with parents/guardians develop child goals usingmultiple sources of data, including but not limited toscreening/assessment results, parent/ guardian home visitorobservations, and One Plan/IEP GoalsComplete Family Conference Form within 3 working weeks after theassessment is completed and put in the childs fileOffer copies of screenings, family conference forms, home visitplans, child observations, child health data, health servicesummaries, and referrals after reviewing with parent/guardian anddocument on home visit planIn partnership with parents/guardians, based on developmentalconcerns, initiate and document referral process to appropriateagencies/providersAssist in implementing One Plan or Individual Education Plan (IEP),for children with special needsIn partnership with families and appropriate communityagencies/providers, participate in and ensure documentation of teammeetings, evaluation process, and development of individual plansStaff will document all child development, health, and disabilitydelivery of services and follow upComplete all registration paperwork with enrolled families withinprogram timelineComplete a strength-based family assessment twice a year with allenrolled families according to timeline; update as neededIdentify and describe family goals; document follow-up and progressAssess family resources, provide and record all referrals given tofamiliesInitiate and complete, in partnership with parent/guardian, childtransition activities according to program policiesActively participate in program and family/child staffingParticipate in development and implementation of communitypartnership activitiesCore Competencies:
