  Job Overview

  The Dialysis Nurse supports Children's Hospital Colorado (CHCO)mission, vision, and core values. The Dialysis Nurse provides directpatient care for patients suffering from acute or chronic kidney failure inaccordance with applicable scope and standards of practice with thepolicies, values, and mission of the organization. Monitors and managespatients during acute and chronic dialysis treatments to maintain a safeenvironment, optimize outcomes, and maximize available resources.

  Department: Renal DialysisHours per week: 38, eligible for benefitsShift: A combination of 3 12 hour shifts/week or 2 12 shifts and 2 6 hourshifts/week. Rotating Saturdays 1-2 per 8 week schedule and on-callrequirements 1 per week and 1 weekend (Fri-Sun) every 8 week schedule.This position is eligible for relocation assistance, if relocating from 100miles or greater.

  Qualifications

  Education: Bachelor of Science in Nursing (BSN).Experience: One year nursing experience.Certification(s): BLS/CPR from the American Heart Association with atleast 6 months left before expiration is required upon hire.Licensure(s): Registered Nurse (RN) license.

These examples do not include all of thefunctions which the employee may be expected to perform.Responsible for the safe and effective delivery of peritoneal and/orhemodialysis direct patient careDevelops and maintains strong professional relationships withmultidisciplinary healthcare providers to provide individualizedpatient/family careCollects, monitors, and reports on quality of care outcomes andmeasures; provides input into the continuous evaluation and improvement ofinternal quality and safety processes, procedures, nd performance measuresCollaborates with patients, families, and providers to facilitate familymeetings and as needed to establish an effective plan of care based onmutually developed goals and in an environment that fosters shared decisionmaking.Assess patient's and family's unmet health and social needs at home or inother community locations and collaborates with social work and familynavigators as needed.Monitors patients' adherence to plan of care and progress toward goals.Facilities and documents change and/or interventions as neededProvides education and coaching to patients and families to improve health
