A Place to Be Proud.ResponsibilitiesAddresses identified patient and family SDH needs and improve patient/family access to community resources.In collaboration with clinical staff, link patient/family to essential support services including housing, nutrition assistance, employment services, and other needs as identified by the patient/family and clinic team.Assists in conducting follow-up calls to patients and families to confirm linkage with identified community resources and/or services, assess potential barriers, and aids in navigating resources and services. May accompany patients and families to social service appointments.Provides appropriate documentation of patient and family interactions in EPIC.Establishes and maintains close relationships with housing resources, food pantries, utility companies, government agencies, treatment facilities and other community-based organizations.Assists NCH clinical staff, supervisor, and organizational partners in the decision-making process for improving linkage of patients/families with community resources.Collects and report data on program activities.Reviews and recommends potential resources that are culturally sensitive to the targeted community.Remains informed of trends, developments, as well as state-of-the-art prevention efforts and strategies, and general information regarding the targeted community.QualificationsKnowledge, Skills, & AbilitiesEducation:High School Graduate or equivalent required.Associates or Bachelor's degree preferred.Certifications:Community Health Worker certification by the Ohio Board of Nursing required or Completion of the Community Health Worke
