Community Health Worker (CHW) - Posting #26009
Hourly Rate: $20.00
Position Summary:
Full-time CHW available for the Care Management Program in Everett, WA. The main function of the Community Health Worker is to support the Transition of Care RN. Working under the Transition of Care Nurse, the CHW’s focuses on Sea Mar patients who have been admitted with a true medical problem and who are going home to self-care. The focus is to oversee the patient population that is falling through the cracks and is at highest risk for readmission.
Exclusions to eligibility include planned surgeries, pregnancy related admissions, primary psych diagnosis, and primary substance use diagnosis. Understanding the program and its exclusions does get a bit complicated in that those exclusions apply to the PRIMARY diagnosis only. For example, if someone with substance use is admitted with COPD, they would be eligible for the program. Additional information will be given once hired for this position.
This position requires flexibility, initiative, and the ability to manage time, resources, and client caseload efficiently. Under the direction of the Transition of Care RN & Social Worker, may follow up on clients in their homes and deliver basic health messages, and ensure that utilization of medical care and other community resources are met.
Qualifications and/or Education:
High School diploma or GED required.
One year of health and/or social services experience preferred.
Bilingual English/Spanish preferred but not required
Background in Community Outreach or Case management preferred
Knowledge of Community Resources preferred
Willingness to commute to meet with clients various settings including clients home, clinic, hospital and/or clinic
