JOB REQUIREMENTS: Job Summary Provides case management through CLTSSupport and Service Coordination to children who are diagnosed withphysical, emotional, or cognitive disabilities. The goal of the CLTSWaiver Program is to support children with substantial needs, as well astheir parents/guardians, by delivering services to promote the goal ofevery child living their best possible life now and into the future.*This position may be eligible for a hybrid schedule to telework up to3 days a week after successful completion of an initial employmentperiod. Essential Duties Support and Service Coordination assists achild or youth and his or her family to achieve an inclusive,independent, and self-empowered life. Support and Service Coordinatorsuse their knowledge of available programs, resources, and services tocreate an individualized service plan that best supports the child andfamily in pursuit of their desired outcomes. This includes connectingfamilies to supports throughout the community and promotes familymembers\' self-determination and involvement in all facets of communitylife. At a minimum, monthly collateral contacts and direct contact withfamily every three months must occur. Assisting to establish andmaintain all aspects of program functional and financial eligibility.Provides participant and family education, support, and advocacyregarding their disability. Assessing and periodically reassessing thechild\'s or youth\'s health, safety, and functional capacity. Reportsincidents that create a significant risk or services harm to thephysical, mental health, safety, or well-being of a child to the WIDepartment of Health Services. Assures the application of client rights,grievance and dispute resolution processes and procedures. Completesauthorizations for purchase of service and obtains supervisory approvalin line with service plans. Monitors contracted services to assure highquality and the appropriate extent of services provided. Interfaces withhealthcare providers/insurers/Medical Assistance/Social Security tocoordinate services. Assists families with transition to adulthoodprocess and connecting them to resources related to needs. Identifiesthe need for and assists in the planning and development of new serviceresources as necessary. Complies with applicable county, state andfederal guidelines related to the provision of service. Completes andmaintains accurate and timely documentation according to applicablerequirements, codes, contracts and policies. Facilitates collaborationand maintains positive working relationships with and betweenparticipants, informal support systems, co-workers, healthprofessionals, courts, legal representatives, contracted providers andthe public at large. Attends and contributes to staff meetings andoverall unit performance. Functions as part of treatmentteams/multi-disciplinary teams to ensure continuity of care.NON-ESSENTIAL DUTIES: Performs related functions as assigned. *****OTHER EXPERIENCE AND QUALIFICATIONS: Knowledge of case managementprinciples and methods, family dynamics, human development, andbehavior. Knowledge of dynamics involved in community-based care forpersons with long-term care needs. Knowledge of social work assessmentprinciples and techniques. Knowledge of client rights, grievance anddispute resolution processes and procedures. Knowledge of state andfederal laws, codes, procedures, and policies pertaining to Medicaid andWisconsin Long-Term Care program delivery. Knowledge of communityresources. Knowledge of needs and dynamics of those with all types ofdisabilities. Ability to establish priorities for service coordinationand to consider alternative treatment plans. Ability to workindependently. Ability to use receive and provide feedback. Ability tobe decisive and to make sound decisions under the pressure of urgentsituations. Ability to plan and organiz work to meet deadlines and tomake effective use of time. Ability to establish and maintain positiverelationships with consumers and their family members, staff, contractedproviders, and the public. Ability to communicate effectively bothorally and in writing. Ability to work the required hours of theposition, which is 37.5 hours weekly, with core hours being8:00am-4:30pm.
