Description

　　We are looking for a dedicated Social Worker to join our Social Work Department and interdisciplinary treatment team at our award-winning Resnick Neuropsychiatric Hospital. This position is on the inpatient adult psychiatry units and geriatric psychiatry units where patients are hospitalized with symptoms and behaviors related to psychiatric illness and neurocognitive disorders.

　　This role involves creating and enacting individualized plans to ensure a safe transition from psychiatric hospital back into the community.

　　Key Responsibilities include:

　　Conduct comprehensive mental health and psychosocial assessments to understand the patients' unique stories and needs.

　　Create personalized post-discharge care plans, focusing on social drivers of health to ensure successful aftercare. Advocate for patients' access to necessary community services.

　　Facilitate individual, group and family therapy sessions. Provide psycho-education to family.

　　Collaborate with interdisciplinary team during treatment rounds. Communicate with community team for continuity of care.

　　CSW 2 salary range: $44.19 - $52.79 Hourly

　　CSW 3 salary range: $48.03 - $57.40 HourlyQualifications

　　Required:

　　Master's degree in Social Work or Social Welfare from an accredited institution.

　　Licensed Clinical Social Worker (LCSW) preferred. LCSW eligibility within 1 year required.

　　Knowledge of psychiatric and neurocognitive disorders and related treatment modalities.

　　Ability to teach patients and families information about psychiatric illness and treatments.

　　Experience providing evidence-based treatment interventions for mental health.

　　Knowledge of community resources for adult and geriatric populations.
