DescriptionThis is a 7 on, 7 off Nights & Weekends shift position at St. John's Medical CenterPosition SummaryThe Clinical Pharmacist is responsible for the safe, effective, and cost-effective use of medications through the application of specialized knowledge, skills, and professional judgment. The pharmacist serves as a patient advocate when planning and implementing pharmaceutical care by considering the patient's disease conditions, background, need for understanding and participation in drug therapy in a fiscally responsible manner. These activities occur in all aspects of pharmacy practice including adult and pediatric inpatient areas, operating room pharmacies, ambulatory oncology and outpatient services (ie., Bolwell Pharmacy). The pharmacist is legally responsible for the review and approval of all activities related to the dispensing of patient specific pharmaceuticals pursuant to a valid prescription or medication order from a practitioner as defined in ORC 4729, 4715, 4731, and/or 4741.Essential FunctionsAccurate order verificationAccurate dispensingMinimize order turnaround timeMedication monitoring and documentationDocumentation of interventionsDocumentation and performance of medication reconciliationInventory control and adherence to Hospital Formulary policiesAdherence to Hospital and Departmental Policies and ProceduresRequired For All JobsPerforms other duties as assigned.Complies with all policies and standards.For specific duties and responsibilities, refer to documentation provided by the department during orientation.Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
