JOB REQUIREMENTS: Overview May be involved in Phase 1 CardiacRehabilitation by providing education and ambulation services topatients in the hospital setting. Responsible for timely referralfollow-up and intake scheduling. Responsible for new patient orientationand assessment and subsequent treatment plan development. Consults orcollaborates with other team members as needed. Serves as a case managerfor a group of patients and is responsible for the application of thetreatment plan, treatment plan modifications, timely progress reporting,appropriate and timely communication with other clinical staff orproviders, outcome tracking, and post program assessment and dischargeplanning. Monitors patients during rehab sessions: blood pressure,pulse, EKG, signs and symptoms, perceived exertion, blood glucose pointof care testing. May be involved in facilitating group education. Maycase management patients enrolled in the weight management programinvolving exercise/fitness assessment, prescription, exercise sessions,and follow-up. May be involved in supervising an adult group exercisesessions. May be involved in conducting fitness testing for occupationalhealth. May be involved in assisting with student orientation andtraining. May be asked to help with community outreach efforts. Attendsmonthly department meetings and patient round meetings. ResponsibilitiesPatient assessment/monitoring: biometric data, EKG, medicalrecord/patient history review, medication reconciliation, signs andsymptoms, exercise tolerance, mental health status, quality of life,dietary habits. Exercise testing: submaximal and exercise tolerancetesting, muscular strength and endurance, flexibility. Exerciseprescription and treatment plan development. Treatment plan/exerciseimplementation. Identifies needed exercise modifications. Makes changesto the treatment plan per the changing needs of the patient. Exercisetraining and leadership: communicates proper execution of the exerciseand use of equipment, communicates safety strategies. Education andbehavior change: identifies educational needs, communicates healthinformation, facilitates attendance to group education classes, patientgoal setting and engagement, uses strategies like motivationalinterviewing and active listening to facilitate successful behaviorchange. Documentation: documents and report patientas clinical statusand response to exercise training in the medical record. Completesregular progress reporting to referring providers and medical director.Environment: maintains safe environment by participating in infectioncontrol procedures and reporting any equipment malfunctions orenvironmental concerns in a timely and appropriate manner.
