　　Primary Responsibilities Include:

　　-Assist residents with personal care including oral/denture hygiene, bathing (bed bath, Century Tub, and/or shower), foot care, nail care, hair care, shampooing, shaving, grooming, and dressing.-Transfer, ambulate, and transport patients-Provide active/passive range of motion with daily care-Assist patients with toileting needs and peri/catheter care-Maintain resident rooms in an orderly fashion-Serve meals, give nourishment and fluids, and assist in feeding residents according to personal needs-Check vital signs including temperature, pulse, respiration, and blood pressure-Report any unusual vital signs to nursing supervisor-Clean and maintain equipment and utility rooms as assigned-Complete documentation including ADL monitoring, special device records, I&O, toileting schedules, intake studies, and BM records in Care Tracker system-Observe resident behavior and report changes or unusual findings to Charge Nurse-Assist residents in preparing for doctors visits, religious services, rehabilitation services, diagnostic services, and social events-Turn and change position of residents, lift as needed-Ability to accompany residents on outings or medical appointments-Other related duties assigned by supervisor(s)according to needs.-Checks vital signs as directed - vital signs include temperature, pulse, respiration, and blood pressure.-Observes resident behavior and report any changes or unusual findings to Charge Nurse.
