General Description
Provide supportive case management services to individual living with HIV/AIDS within multiple counties surrounding and including Monroe County, ensuring client’s access to and maintenance of medical care.
Essential Duties and Responsibilities
Complete initial intake and assessment paperwork for eligible participants and obtain required AIDS Institute eligibility documents to ensure grant qualified eligibility.
Provide case management, including completing reassessment and obtain essential documents within required time frame.
Assist with engaging clients into care and treatment to improve their health outcomes and achieve viral suppression.
Address and work to remove barriers to enable client access to all necessary components of health care, including but not limited to mental health and substance use services.
Maintain high quality and timely documentation in electronic data bases, including but not limited to AIRS.
Submit copies of electronic data base entries to supervisor bi-weekly to ensure accuracy.
Work collaboratively with ESS team to engage clients to receive appropriate care, treatment, and supportive services with the overall client centered goal of viral suppression and self-management.
Meet with each client and maintain telephone availability for clients based on client need (minimum weekly contact.)
Conduct and/or participate in multidisciplinary case conferences; coordinate services and referrals with both service and clinical providers that facilitate a client’s engagement and retention in care.
Oversee implementation of service plans to the satisfaction of the individuals served.
Assist individuals with accessing services for medical care, financial benefits, housing and psycho-social needs.
Provide client with information, support, advocacy, referral and linkage to services to promote independence and self-sufficiency.
Negotiate and advocate on behalf of clients for services that support independence; the overall goal for clients will be to transition to Health Homes or another provider.
Participate in collaboration meetings, team meetings, AIDS Institute provider meetings, and case conferences.
Assist in completing mandatory and required reports for program by funder and supervisor respectively.
Provide crisis intervention as needed with clients.
Represent Catholic Charities Community Services on appropriate boards and committees as assigned or approved by supervisor.
Participates in relevant Agency meetings and/or trainings
Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.
Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agency's Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse

Qualifications
Education: B.A. or B.S. Degree, with 2 years of experience working in the filed of HIV/AIDS, behavior health, substance abuse, health education, or other chronic illness.
Cultural and linguistic competence for the target population. If position requires bilingual skills to be used, additional compensation will be offered. A combination of education and experience may be substituted at the discretion of the Executive Director.

Additional Requirements:
Familiarity with regional HIV primary care, mental health, substance abuse and other services and resources.
Possess an understanding of community level work and the importance of collaborating and coordinating with other organizations.
