ThisRN Case Manager Coordinatorwill coordinate and manage thenursing services of home care clients through development,implementation and evaluation of a plan of care, working incollaboration with the interdisciplinary team, specifically ouragency\'s Visit Nurses, and ensure that services are completed atquality levels consistent with professional standards of practice. Incollaboration with the patient/caregiver, completes patient plan of careand submits admission paperwork. Develop, schedule and communicatepatient visit schedules to assigned Visit Nurses and ensure visits, planof care are completed timely and within agency\'s quality standards.Accurately and timely performs ongoing assessments of patients asevidenced by visit documentation, follows physician orders and plan ofcare revisions based on assessment findings. Identify patient goals anddevelop a plan and implement interventions. Ensure goals are clearlydelineated and prioritized on the patient plan of care and addressed byall team members Case conferencing with team members occurs regularlywhich includes creating PCW/HHA Care Plans, initiating and updatingPlans in accordance with patient needs and Agency policy. Plan of careis revised on a regular basis reflecting changes in patient\'s on goingassessment and /or progress or lack of progress toward goals. Informphysician, clinical manager and interdisciplinary team members ofchanges in patient status as evidenced by Case Communicationdocumentation.
