Homes for the Homeless, Inc. (HFH) is a non-profit social services organization operating family shelters throughout NYC that serve as a hub of opportunity for families both residing in shelter as well as those in the neighboring community. On-site programs range from childcare, early childhood education centers, afterschool clubs and programs, and youth recreation activities, such as sports teams, to housing assistance, case management, parenting workshops, career and job vocational training, support groups and substance abuse assistance, and family engagement and recreation events for the whole family. ABOUT THIS ROLE
Homes for the Homeless is searching for Case Managers to work with families with children residing in at a new shelter facility opening in Hell's Kitchen, to help them set and achieve goals, connect to vital services and benefits, and assist families in working to achieve self-sufficiency and a return to permanent housing. This position requires strong organizational skills, attention to detail, sensitivity for issues surrounding poverty, and good written and verbal skills. In addition to in-person meetings and liaising between families and other service providers, the Case Manager will also maintain and update case files to document progress and facilitate information sharing with other staff members. Bi-lingual (English/Spanish) preferred.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Handle a caseload of approximately 22 homeless families, maintaining professional relationships and client confidentiality
Conduct initial intake interviews and assessments with families arriving at the facility
Collaborate with families to develop service plans based on an assessment of each family’s needs, updating plans on through regular meetings with the family.
Assist families with referrals to outside agencies for employment, education, parenting, legal, medical, and mental health services.
Connect families to public benefits and assist families in moving to permanent housing.
Maintain and update case files to document progress
QUALIFICATIONS
Bachelor’s degree in Social Work or a related field.
Familiarity with case management, entitlement systems, and a knowledge of issues that impact families is a plus.
Proficiency with Microsoft Office.
Bilingual in Spanish preferred.
