As a Case Manager, you will support and provide quality services and crisis intervention to youth as they transition from homelessness to housing stability. You will be part of a team of staff and volunteers working with youth and youth headed households as they identify and work toward achieving their individual goals. You will have opportunities to assist the youth in increasing their independent living skills - both formally and informally - through assisting with facilitation of weekly skills groups and individual interaction. You will help them assess and retain community supports (medical, mental health services, etc.) and coordinate services. You will participate in community activities with the youth. You are required to maintain documentation, perform record keeping and participate in periodic meetings/trainings. Outreach will be a requirement to learn about the different community organizations, programs and to capture youth attention. Job Requirements: Bachelor's degree in social service or related field and one year of working with youth, life skills training, and group facilitation. A valid driver's license, acceptable driving record, reliable transportation, valid insurance and a Motor Vehicle Check. Strong organizational skills. Ability to be focused and attentive for an extended period of time. Able to develop, guide and provide follow-up for client transition plans. Ability to promote a positive attitude regarding LSS and other programs to the public. Knowledgeable regarding community programs and referrals.
