Our goal is to provide our residents with temporary emergency housing and services that can prevent a future crisis, thereby improving the entire community. Case management is specifically focused on success in housing, developing case plans, connecting with appropriate community resources, and supporting clients' individual housing goals.Provide county-based outreach to connect the homeless individuals to screening and diagnostic treatment services, habilitation and rehabilitation services, community mental health, substance use treatment, case management services, referrals for primary healthcare, job training, educational services, and housing services.Adherence to Peak of Hope's philosophy of service delivery is required, including:• Ability to hold residents in unconditional positive regard and provide services without judgment.• Deep respect for clients' autonomy and inherent right to make their own choices about their own lives, while also helping them understand and navigate those choices.• Proactive and solutions-focused approach to person centered case management with the goal of attaining self-sufficiency and/or housing stability.Responsibilities: • Case Manager will be the point of contact (access point) for all homelessness, including but not limited to those who have a disability of a mental illness and/or alcohol drug diagnosis in Logan County • Work with each resident to develop goal plans according to individual goals and objectives with formal reviews• Maintain contact with all residents on a scheduled basis, appropriate to their status and needs. Minimum of 1 formal meeting to include case plan and case review and at least 1 informal with basic follow-up (weekly);• Ensure resident interactions (in person or by phone) are person-centered, goal-focused and reflect each person's goal plan.• F
