Assessing patients physical and mental wellness, needs, preferences, andabilities and using these to develop tailored treatment plans.
Working with patients, family and friend support networks and healthcare professionals to put care plans in place.
Listening to patients concerns and providing counseling or interventionas required
Recording patients progress including charting referrals, home visitsand other notable interactions
Evaluating patients progress periodically and adjusting as needed toimprove outcomes.
Following up with discharged patients to ensure they were satisfied withservices, and they are still in good physical and mental health.
Acts as the patients liaison; helps patients make informed decisions byacting as their advocate regarding their clinical status and treatmentoptions.
Assists in the arrangement of transportation for patients and accessingneeded and available resources.
Coordinate and provide care that is safe, timely, effective, efficient,equitable, and client centered.
Handle case assignments, draft service plans, review case progress anddetermine case closure.
Helps clients achieve wellness and autonomy.
Facilitates multiple care aspects (case coordination, informationsharing, etc.)
Develops effective working relations and cooperates with medical teamthroughout the entire case management process.
Arranges abeyances with social services, health, and governmentalagencies.
Takes the extra mile and interact with patients to keep track of theirprogress and to ensure satisfaction.
Records cases information, complete accurately all necessary forms andproduce statistical reports.
Assesses and address motivational and psychosocial issues.
Adheres to professional standards as outlined by protocols, rules, andregulations.
